Our office will be happy to process, file and battle your insurance company on your behalf in order that you receive all eligible benefits outlined in the policy that you purchased. On all treatment that we are asked to accept assignment of your insurance payments, you will be asked to make specific financial arrangements with our office on the full amount of treatment prior to beginning treatment. Our experience shows that there may be a difference in what the insurance company says and what they actually do!. (i.e., post dated checks or credit cards for 3 months on the full amount of care.

1. Pre-Payment Discount
    • 5% discount cash or check paid 48 hours in advance of
       appointment.
    • 3% discount credit card paid 48 hours in advance of
       appointment.

2. Pay in full on day of appointment:
    • 3% discount cash or check

3. Payment of ½ fee at prep appointment and ½ fee at
    seat appointment:

    finance program"
   • Total amount of fee divided into 3 monthly payments
   • 1st payment due at time of preparation or first appt..
   • We arrange this by post-dated checks or past-dated credit
      card charges.

5. Care Credit: 12 months no interest financing

6. Care Credit: 36 & 48 month financing at 12.9%
